Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Meridian Health Services. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare organization.
Job Summary:The LPN will be responsible for providing nursing care services and related functions to patients in our Group Homes. This includes responding to patient requests, consulting with medical staff, and administering medications as ordered. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work effectively in a team environment.
Key Responsibilities:- Respond to patient requests and concerns regarding medications, side effects, and special precautions.
- Phone in prescription orders to pharmacies as directed by medical staff.
- Review lab and test results, reporting concerns and abnormal results to medical staff.
- Obtain medical histories and perform physical assessments as directed by medical staff.
- Provide health screenings and medical triage as needed.
- Administer injectable medications as ordered by medical staff.
- Provide educational groups, inservices, and one-on-one teaching to patients and employees.
- Participate in reviews and modifications of patients' services during treatment planning.
- Current licensure as a practical nurse in the state of Indiana or current licensure as a practical nurse in another state with evidence of application for Indiana licensure.
- Two years of previous LPN experience preferred.
