Clinical Care Specialist

We are seeking a skilled Clinical Care Specialist to provide quality, clinical services to patients in their homes. As a member of our interdisciplinary team, you will deliver patient care within the organization's philosophy, policy, and standards of community health practice.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work independently and as part of a team. A strong understanding of healthcare principles and practices is essential, along with the ability to analyze complex situations and make informed decisions.
About the RoleThis position requires a registered nurse with a minimum of one year of experience in an acute care setting. Prior home care experience is preferred. The successful candidate will be responsible for assessing patient needs, developing and implementing individualized care plans, and coordinating care with other healthcare professionals.
Key responsibilities include:
- Providing skilled nursing care to patients in their homes, including assessment, education, and treatment
- Developing and implementing individualized care plans that meet the unique needs of each patient
- Coordinating care with other healthcare professionals, including physicians, therapists, and social workers
- Monitoring patient progress and adjusting care plans as needed
- Communicating effectively with patients, families, and healthcare teams to ensure seamless care delivery
To be considered for this role, applicants must have:
- A current Massachusetts license as a Registered Nurse
- A minimum of one year of experience in an acute care setting, with prior home care experience preferred
- Excellent communication and interpersonal skills, with the ability to work independently and as part of a team
- A strong understanding of healthcare principles and practices, with the ability to analyze complex situations and make informed decisions
