Job Summary:
We are seeking a skilled and compassionate Registered Nurse to join our team at Assisting Hands Home Care. As a Nurse Care Specialist, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.
Key Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans.
- Monitor and evaluate patient progress, updating care plans as needed.
- Provide education and support to patients and their families on health management and care procedures.
- Collaborate with other healthcare professionals to coordinate patient care.
Requirements:
- Minimum of 1-2 years of nursing experience, preferably in home health or a similar setting.
- Level 2 Background Screening.
