Job Title: Universal Patient Navigator

We are seeking a highly skilled and compassionate Universal Patient Navigator to join our team at Planned Parenthood of Tennessee and North Mississippi. As a key member of our healthcare team, you will play a vital role in navigating patients to various resources, ensuring they receive the highest quality care and support.

Job Summary:

The Universal Patient Navigator will focus on two key areas: Patient Navigation and Case Management. This role will involve guiding patients to resources needed, including services such as abortion care, LGBTQIA support, transportation for healthcare, and housing/lodging. You will work closely with our Chief Clinical Officer, Sr. Manager, Health Centers, Health Center Managers, and Call Center Representatives to ensure seamless communication and coordination.

Key Responsibilities:
  • Advance patient navigation program goals and objectives by identifying and addressing existing gaps and opportunities within existing patient navigation processes.
  • Maintain a strong data collection, tracking, and reporting system to ensure highest-need patients are being identified, routed, and connected to navigational support.
  • Develop and maintain comprehensive resource and referral sheets to connect patients to care and support services.
  • Establish relationships with external stakeholders and organizations to build and strengthen systems and processes that support access to patient support resources.
  • Perform other duties as assigned.
Requirements:
  • Associates or Bachelor's Degree in social work, human services, counseling, psychology, trauma care, or project management.
  • Three (3)+ years of experience in case management, social work, or human services work.
  • Certifications such as CCM, LSW, or CMSW are preferred.
