Job Summary

We are seeking a highly motivated and compassionate Medical Case Manager to join our team at Friends For All. As a Medical Case Manager, you will play a vital role in providing direct case management services to our clients, ensuring they receive the necessary support and resources to manage their HIV/AIDS treatment and improve their overall health and well-being.

Key Responsibilities
  • Provide comprehensive case management services to clients, including assessment, goal setting, and service planning.
  • Develop and implement individualized care plans that address the client's physical, emotional, and social needs.
  • Coordinate with healthcare providers, community organizations, and other stakeholders to ensure seamless delivery of services.
  • Monitor client progress, make adjustments to care plans as needed, and ensure timely follow-up with clients.
  • Document all interactions with clients, including case notes, progress reports, and other relevant information.
  • Collaborate with the healthcare team to ensure clients receive timely and effective medical care.
  • Provide education and support to clients on HIV/AIDS treatment, medication adherence, and healthy living.
  • Identify and address barriers to care, including transportation, housing, and food insecurity.
  • Develop and maintain relationships with community partners to ensure access to resources and services.
Requirements
  • Bachelor's degree in a related field, such as social work, nursing, or healthcare administration.
  • 1-3 years of experience in case management, healthcare, or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and customer service.
  • Proficiency in Microsoft Office and other relevant software applications.
  • Valid driver's license and reliable transportation.
Preferred Qualifications
  • Previous experience working with HIV/AIDS clients or in a healthcare setting.
  • Bilingual or multilingual skills, particularly in English and Spanish.
  • SOAR certification or experience with Social Security benefits.
  • Healthcare Marketplace Certified Applications Counselor certification.
