Job Summary

At Baptist Memorial Healthcare Corporation, we are seeking a skilled RN-Case Manager to join our team. As a key member of our Case Management Admin department, you will play a vital role in ensuring the seamless transition of patients throughout the continuum of care.

Responsibilities
  • Conduct thorough assessments of patients' needs and develop effective care plans to optimize resource utilization and service delivery.
  • Collaborate with healthcare teams to facilitate timely discharges and reduce readmissions.
  • Communicate effectively with payers to ensure compliance with federal and other regulatory requirements.
  • Participate in quality improvement initiatives to enhance patient outcomes and reduce costs.
Requirements
  • Bachelor's degree in Nursing (BSN) or Master's degree in Nursing (MSN) preferred.
  • Minimum of one year of clinical experience as a Registered Nurse (RN).
  • Preferred: RN with three years of clinical experience and Case Management experience in a hospital or payer setting.
Preferred Qualifications
  • Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification.
  • Critical thinking skills, effective communication, and strong organizational abilities.
