Salary: $73,685.10 - $110,527.65

About the Role:
The Hospice Admission RN plays a vital role in coordinating hospital-based hospice and palliative care services. This dynamic individual will receive referrals, arrange admissions, and interface with patients, families, and attending physicians to ensure seamless transitions. With excellent communication skills and attention to detail, this RN will work closely with the interdisciplinary team to develop personalized care plans that meet the unique needs of each patient.

Key Responsibilities:
- Receive and triage incoming referrals for hospice and palliative care services
- Coordinate admissions and interface with patients, families, and attending physicians
- Develop and implement comprehensive care plans in collaboration with the interdisciplinary team
- Provide nursing assessments, judgments, and treatment decisions based on patient needs and wishes
- Supervise LPNs, CNAs, Unit Secretaries, and Volunteers as needed

Requirements:
- Diploma or Associates degree in Nursing from an accredited program (BSN preferred)
- Minimum 2 years of public health/community health, med surg, or related nursing experience (home care, hospice, and oncology experience a plus)
- Licensure from the governing body for practice in the District of Columbia, State of Maryland, and/or Commonwealth of Virginia
- BLS (CPR) certification required for Maryland and D.C. practices

Work Environment:
This role requires working in a fast-paced office setting with computers, printers, copy/fax machines, and telephones. The noise level is generally light to moderate, and the environmental conditions are typical of a temperature-controlled building.
