Social Work Case Manager III

We are seeking a highly skilled Social Work Case Manager III to join our team at Emory Healthcare. As a key member of our care coordination team, you will be responsible for ensuring seamless transitions of care for our patients, facilitating high-quality clinical and cost outcomes, and advocating for patients and families with internal and external stakeholders.
Key Responsibilities- Coordinate patient care from admission through discharge, ensuring smooth transitions and efficient care delivery.
- Identify and participate in the development of strategies to reduce unnecessary length of stay and resource consumption.
- Communicate effectively with patients, families, and care teams to educate them on post-acute services, transitions of care, and readmission mitigation.
- Collaborate with internal and external stakeholders to procure and secure post-acute services, ensuring optimal patient outcomes.
- Participate in process improvement initiatives, sharing thoughts and ideas to enhance care coordination and patient outcomes.
- Masters in Social Work from an accredited institution.
- 3 years of recent healthcare experience, preferably in an acute care setting.
- LCSW license in the state of Georgia.
- Working knowledge of software/EEMR applications.
- ACM, CCM certification preferred.
