Nurse Care Coordinator

Community LIFE is dedicated to providing comprehensive care and support to older adults. We are seeking an On-Call Registered Nurse to join our team.

**Schedule:**

This position requires the ability to arrive at our participants' homes within 45 minutes of receiving a call. The schedule includes on-call shifts from 4pm to 8am Monday-Friday and 24 hours a day on weekends, every other week.

**Key Responsibilities:**

* Troubleshoot, advise, teach, and coordinate care for older adult participants
* Evaluate participants for episodic illness after business hours in collaboration with the Provider
* Document care provided and changes in participant's condition in a timely manner
* Provide nursing care and prescribed treatments
* Demonstrate nursing care to participants and caregivers/families

**Required Skills and Qualifications:**

* Graduated from an accredited nursing school
* Minimum of one year of experience working with a frail or elderly population, preferably in long term care, home care, or community health
* Experience in teaching patients and other healthcare workers is preferred
* Registered Nurse licensed in the state of Pennsylvania
* Possess a valid PA Driver's License
