Job Title: Social Services Floater

At Clinical Care, we are seeking a highly skilled and compassionate Social Services Floater to join our team. As a key member of our staff, you will play a vital role in providing exceptional support to our patients and their families.

Job Summary:

The Social Services Floater will travel to various centers as needed, providing general assistance to our patients, completing processes and applications to receive benefits through local resources and government-assisted programs.

Key Responsibilities:
  • Assist patients with the completion of applications for SNAP, Medicaid, Medicare Saving Program, or other government benefit programs, based on the information provided and with the submission of verifications and general information regarding the application process.
  • Assist patients in making calls related to benefits and provide printer, fax, and copy machine for application-related documents.
  • Help patients and visitors activate or request their Electronic Benefits Transfer card (EBT).
  • Assist patients with the application of Medicare, Supplemental Security Income (SSI), social security card, change of status, or other benefit-related to Social Security Administration (SSA).
  • Assist patients in completing the naturalization process application and follow up until completion.
  • Complete American Passport application and make the appointment to the corresponding office.
  • Assist patients with the application of government cell phones, affordable high-speed low-cost internet, Home Energy Assistance Program (LIHEAP), housing, Section 8, or any other public assistance benefit.
  • Assist patients with the application for special transportation services, bus pass application, or disabled person parking permit.
  • Participate in monthly department meetings and service training to maintain standards certifications.
  • Handle any complaints from patients and family members as appropriate.
  • Complete all processes and handle all confidential information in an honest, ethical, and professional manner.
  • Ensure that all HIPAA standards are met.
  • Travel to various centers as needed.
  • Perform other duties as assigned.
Qualifications:
  • High school diploma, equivalent, or relevant experience required.
  • Previous experience with any government-assisted programs and immigration services.
  • Bilingual in English and Spanish preferred; must be able to read, write, and speak English.
  • 1-2 years of experience in a customer service environment preferred.
  • Basic computer knowledge including Microsoft Office, internet, document with electronic health records.
  • Completes applications and documents with minimal errors, sending e-faxes and email.
  • Strong people skills, proactive, independent decision-making, and customer service-oriented.
  • Excellent communication, customer service, and telephone skills.
  • Cheerful, pleasant, knowledgeable, and professional demeanor.
  • Strong organizational skills and ability to multi-task effectively.
  • Independently work and relate well with all levels of individuals within the organization.
