Position Overview
The Licensed Practical Nurse (LPN) operates under the supervision of a Registered Nurse or Nurse Manager, adhering to the LPN Scope of Practice. The LPN is dedicated to delivering comprehensive care to patients, addressing their physical, emotional, developmental, psychosocial, and educational requirements. This role encompasses the coordination and provision of high-quality care, which includes preparing patients for examinations, assisting in treatments, coordinating diagnostic tests, and documenting care in medical records. The LPN also plays a crucial role in patient and family education and is expected to engage in staff meetings, in-services, special projects, and ongoing education. Additionally, the LPN may serve as a mentor for newly hired colleagues.
Key Responsibilities
DIRECT PATIENT CARE
- Evaluates and assists in managing the comprehensive needs of patients in accordance with physician directives, under RN supervision, utilizing the nursing process.
- Executes routine nursing tasks to fulfill patient needs under RN oversight.
- Adheres to established productivity benchmarks.
- Demonstrates proficiency in accessing and de-accessing ports, including flushing lines and maintaining sterile techniques.
- Exhibits knowledge and skill in medication administration as defined within the organizational scope.
- Seeks guidance, training, or direction from leadership as necessary.
- May assign tasks to unlicensed personnel, as appropriate and within their scope of practice.
- Provides patient-centered care that is respectful of individual characteristics (e.g., age, gender, health conditions) in a culturally sensitive manner.
- Collaborates with patients, their families, and significant others to create a safe and comfortable environment for care delivery, prioritizing the patient's goals in the care plan.
- Coordinates care and services with the RN and other healthcare team members.
- Notifies the RN, physician, or clinical supervisor of any changes in the patient's condition as appropriate.
- Works with the RN to develop and adjust individualized care plans based on patient needs and physician orders.
- Acts as an educator for patients to promote self-care and support for caregivers.
- Utilizes teach-back methods to ensure understanding of self-care practices by patients and/or caregivers.
- Participates in the orientation and training of new staff and nursing students as appropriate.
- Responsible for attending continuing education and in-service training as required.
- Maintains current clinical knowledge relevant to the care of oncology patients.
- Ensures accurate and timely completion of required documentation.
- Demonstrates knowledge and appropriate application of regulatory standards.
- Responsible for managing communications, including returning patient calls and checking work emails regularly.
Education: Practical Nursing Diploma Required
Licenses and Certifications: All certifications must be obtained upon hire unless otherwise specified.
- Licensed Practical Nurse
- Basic Life Support (BLS) Certification
Skills: Strong leadership, problem-solving, organizational, and time management skills. Proficient assessment and clinical skills. Excellent written and verbal communication abilities.
