Job Summary

Quality Moments Behavioral Health is seeking a highly skilled QMHS to provide facility-based services that encourage and support progress towards meeting client goals. The ideal candidate will coordinate and provide care that is safe, timely, effective, efficient, and client-centered.

The QMHS will work closely with licensed staff members to provide quality behavioral health services as needed. This includes interacting with clients and their families to keep track of their progress and to ensure satisfaction, recording case information, and accurately completing all necessary forms and paperwork while adhering to agency standards.

The QMHS will also assess and address motivational and psychosocial issues, facilitate multiple care aspects including case coordination and information sharing, create service plans, and assist in case closures. The ideal candidate will maintain a flexible schedule which may include some evening and weekend availability.

Key Responsibilities:

  • Provide facility-based services that encourage and support progress towards meeting client goals
  • Coordinate and provide care that is safe, timely, effective, efficient, and client-centered
  • Work closely with licensed staff members to provide quality behavioral health services as needed
  • Assess and address motivational and psychosocial issues
  • Facilitate multiple care aspects including case coordination and information sharing
  • Create service plans and assist in case closures

Requirements:

  • Experience in social work, counseling, human services, or related field or a minimum of 3 years' experience working with older adults in a professional and supervisory capacity
  • Strong organizational skills, interpersonal skills, judgment, flexibility, commitment, and internal drive/motivation
  • Valid driver's license required
  • Automobile insurance with minimum state liability amounts required
