Position Overview:
As a Family Support Services Coordinator, you will leverage your empathy, expertise, and communication abilities to make a meaningful impact. Pathways Health and Community Support, LLC. is a part of the national Pathways network, dedicated to providing mentoring, community-based interventions, crisis support, and outpatient counseling services aimed at assisting children, families, and adults in a respectful and effective manner.
Role Responsibilities:
In this role, you will be responsible for:
- Providing essential intervention and managing client cases, acting as a vital resource for families in need.
- Assisting in the case management of children, taking an active role in their treatment planning.
- Collaborating with treatment teams to offer insights and develop comprehensive care plans.
- Advocating for individuals facing challenges, connecting them with organizations that can enhance their circumstances.
Qualifications:
The ideal candidate will possess:
- A Bachelor’s degree in a Human Services field from an accredited institution.
- CPR and First Aid certification, or a willingness to obtain certification prior to client interaction.
- Experience working with children and adolescents in therapeutic, community-oriented settings.
- The ability to work flexible hours to meet program demands and serve multiple locations.
- Exceptional organizational, communication, mentoring, and teamwork skills.
- A sensitivity to diverse cultures and socioeconomic backgrounds.
