Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at Phoenix Children's Hospital. As a key member of our pharmacy department, you will play a critical role in ensuring the safe and effective use of medications for our patients.

Key Responsibilities
  • Review and manage medication orders for therapeutic appropriateness, taking into account patient age, weight, height, laboratory values, diagnosis, and other relevant clinical information.
  • Screen for and manage clinical alerts/conditions, such as allergies, interactions, and adverse drug reactions.
  • Actively problem-solve issues with medication orders and contact prescribers to recommend alternative medication therapy as needed.
  • Comply with the directives of the Pharmacy and Therapeutics Committee related to Formulary Management.
  • Document clinical interventions to meet or exceed established cost containment and evidence-based medication therapy goals.
  • Participate in Main Hospital Core and Remote Campus Pharmacist assigned duties.
  • Act as liaison between the pharmacy and medical and nursing staff for issues pertaining to medication management and pharmacy services.
Requirements
  • Registered Pharmacist licensure in the state of Arizona.
  • Experience in pediatric pharmacy practice, preferably in a hospital setting.
  • Strong clinical skills and knowledge of pediatric medications.
  • Excellent communication and problem-solving skills.
  • Ability to work effectively in a fast-paced environment.
