Job Description
As a Home Health Registered Nurse, you will provide high-quality nursing care to patients in their homes, focusing on case management and skilled nursing visits. You will work closely with physicians and other healthcare professionals to develop individualized care plans, ensuring that patients receive the best possible care.
You will be responsible for:
- Providing admission, case management, and follow-up skilled nursing visits for home health patients
- Administering ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager
- Conferring with physician in developing the initial plan of treatment based on physician's orders and initial patient assessment
Required Skills and Qualifications
To succeed in this role, you must have:
- A diploma, associate or bachelor degree in nursing
- A minimum of one year nursing experience in skilled home visits
- OASIS proficiency required for Weekend Baylor position
- Current and unrestricted registered nurse licensure
- Current CPR certification
- HCHB (HomeCare Home Base) experience preferred
