Job Summary:

We are seeking an experienced Registered Nurse to join our team as a Skilled Home Visits Registered Nurse Leader. As a key member of our care team, you will provide comprehensive nursing care and support to patients in the comfort of their own homes, working collaboratively with healthcare personnel to ensure seamless transitions and effective care coordination.

**Key Responsibilities:**
• Deliver high-quality, patient-centered care, addressing the unique needs and goals of each patient.
• Collaborate with healthcare personnel to develop and implement individualized care plans that meet the specific needs of each patient.
• Conduct regular assessments and evaluations to ensure the effectiveness of care plans and identify areas for improvement.
• Provide education and support to patients and their families, empowering them to manage their care and achieve optimal outcomes.
• Maintain accurate and timely documentation of patient information, ensuring compliance with regulatory requirements and industry standards.

Requirements:
• Diploma, Associate or Bachelor Degree in Nursing
• Minimum of two years nursing experience in Skilled Home Visits
• Proficiency in OASIS required for Weekend Baylor position
• Current and unrestricted Registered Nurse licensure
• Current CPR certification
• HCHB (HomeCare Home Base) experience preferred
• Valid driver's license, auto insurance, and reliable transportation
