**Responsibilities:**
• Provide comprehensive nursing care to patients, including case management and skilled nursing visits.
• Collaborate with healthcare personnel to coordinate patient care and ensure seamless transitions between services.
• Develop and implement individualized care plans that address the unique needs and goals of each patient.

Requirements:
• Diploma, Associate or Bachelor Degree in Nursing
• Minimum of one year nursing experience in Skilled Home Visits
• Proficiency in OASIS required for Weekend Baylor position
• Current and unrestricted Registered Nurse licensure
• Current CPR certification
• HCHB (HomeCare Home Base) experience preferred
• Valid driver's license, auto insurance, and reliable transportation
