Care Management Support Assistant 3

About the Role:
The Care Management Support Assistant 3 plays a vital role in the administration of care management at CenterWell. This position provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state. The Care Management Support Assistant 3 guides members/families toward and facilitates interaction with resources appropriate for the care and wellbeing of members.
Key Responsibilities:
- Provides non-clinical support to the assessment and evaluation of members' needs and requirements.
- Guides members/families toward and facilitates interaction with resources appropriate for the care and wellbeing of members.
- Performs advanced administrative/operational/customer support duties that require independent initiative and judgment.
- May apply intermediate mathematical skills.
Requirements:
- 2+ years of prior experience in a Healthcare setting providing support to patients and assisting them with enrolling in community resources.
- MUST be Bilingual in English and Spanish.
- Knowledge of Microsoft Word, Excel and Outlook.
- Administration support experience in a healthcare industry.
- Familiarity with care and well-being resources.
Preferred Qualifications:
- Associate's Degree.
- Microsoft Access proficiency.
- Knowledge of Medical Terminology.
