Element Care is seeking a skilled Behavioral Health Therapist to join our team. As a Behavioral Health Therapist, you will be responsible for providing behavioral health services to clients at assigned sites, including those in nursing home settings.
Key Responsibilities- Provide Behavioral Health Services: Deliver behavioral health care to a caseload of clients referred by the site's Primary Care clinicians and behavioral health APRNs.
- Consult with Interdisciplinary Team: Provide consultation to the Interdisciplinary Team regarding behavioral health issues.
- Perform Psychiatric Evaluations: Conduct routine and urgent psychiatric evaluations.
- Develop Treatment Plans: Develop clinically appropriate behavioral health treatment plans with emphasis on relational, behavioral and affective functioning.
- Implement Evidence-Based Interventions: Implement evidence-based and best practice psychotherapeutic interventions.
- Monitor Client Progress: Monitor clients' response to interventions and adjust plans, as necessary.
- Document Interventions: Document interventions and rationale/response in the Electronic Health Record (EHR).
- Provide Behavioral Health Consultation: Provide behavioral health consultation and support to the Interdisciplinary Team to increase the ability of team members to integrate psychiatric concepts/skills into the delivery of care.
- Participate in Program Projects: Participate in Behavioral Health program projects to enhance clinical practice, teamwork and program development.
- Cover for Colleagues: Cover for Behavioral Health colleagues when needed.
- Master's or Doctoral Degree: Master's or doctoral degree in social work, counseling or psychology.
- License to Practice: License to practice in the Commonwealth of MA as an LICSW or LMHC.
- Experience in Psychotherapy: At least 2-5 years of experience in the practice of psychotherapy.
- Experience with Older Adults: At least 1 year of experience with elders.
- Covid Vaccines: Covid vaccines required.
