Pradera Corporation, a division of APM, has a rich history in the child welfare sector, providing comprehensive, high-quality, and trauma-informed services. Our mission is to foster healthy lifestyles, safeguard vulnerable children and youth, and empower them to express their voices within their communities.
POSITION OVERVIEW:
The Adoption Services Coordinator (ASC) will ensure adherence to state and county regulations regarding adoption for children, youth, and families referred to APM through the Statewide Adoption and Permanency Network (SWAN). The ASC will manage casework related to adoption services for children/youth, adoptive resource parents, and foster parents, in alignment with service specifications and contract obligations with the Philadelphia Department of Human Services (DHS).
Key Responsibilities:
- Evaluating the living conditions of adoptive resource homes
- Conducting monthly visits to Adoption Finalization recipients, children, and youth until the finalization process is completed to ensure the needs of the adoptive resource parent and child/youth are addressed
- Gathering and organizing child/youth information from diverse sources to prepare for Child and Family Profiles, Child Preparations, and Finalizations
- Maintaining accurate and current files in compliance with state and county regulations, providing updates on families and services
- Collaborating with a multidisciplinary team to deliver SWAN services
- Completing required reports, documentation, and benchmarks within specified timeframes as per Adoption and Foster Care contract requirements
- Supporting adoption training and social work services for adoptive families
- Preparing for and participating in permanency conferences, child preparation plan meetings, court sessions, and other necessary events

Qualifications:
- EDUCATION: Bachelor's Degree in Social Work, Psychology, or a related field
- EXPERIENCE: At least one year of experience in the adoption system or a similar program involving direct social service delivery, preferably within the Hispanic community.
- CLEARANCES: Pennsylvania Child Abuse Clearances, Pennsylvania Criminal Record Clearance, Federal Bureau of Investigation (FBI) background check, and Medical Examination
- VALID DRIVER'S LICENSE: Required
