Job Summary

Aveanna is seeking a skilled Physical Therapist Assistant to join our team in providing high-quality care to patients in their homes. As a Physical Therapist Assistant, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote optimal patient outcomes.

Key Responsibilities
  • Provide skilled physical therapy services to patients in their homes, as directed by the physician and in accordance with the plan of care.
  • Communicate effectively with patients, families, and caregivers to ensure seamless care transitions and optimal patient outcomes.
  • Collaborate with the interdisciplinary team to develop and implement individualized plans of care that address the unique needs of each patient.
  • Conduct thorough assessments of patients' physical and functional abilities, and develop and implement plans to improve their mobility and function.
  • Provide education and training to patients and caregivers on safe use of prosthetic, orthotic, and assistive devices.
  • Document all patient interactions, assessments, and interventions in a timely and accurate manner.
Requirements
  • Graduation from a Physical Therapist Assistant education program from an accredited university/program.
  • Current, unrestricted state license as a Physical Therapy Assistant in the state of practice.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.
  • Valid CPR and State Drivers License.
