Home Health Physical Therapist Position

We are seeking a skilled Physical Therapist to join our team in providing high-quality care to patients in their homes. As a Physical Therapist with LHC Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities:
The Physical Therapist will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment and evaluation, the Physical Therapist will work to determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate the patient's progress.
Requirements:
Current Physical Therapy licensure in the state of practice is required. A current CPR certification and a valid driver's license with vehicle insurance are also necessary. The ideal candidate will have a strong passion for delivering high-quality patient care and a commitment to ongoing education and professional development.
