Job Summary

We are seeking an experienced Nurse Practitioner to provide primary care to homebound and palliative care patients in the Warren County, NJ region.

You will work under the supervision of a physician to care for a diverse set of patients with chronic diseases, applying medical techniques and principles to deliver high-quality care.

Key Responsibilities
  • Provide high-quality care to primary and palliative patients in and around Warren County, NJ
  • Assess patient health through physical examinations, medical histories, and creating personalized care plans
  • Effectively communicate with office staff and clinical operations across various internal platforms
  • Provide patient-centered care to a panel of patients, developing impressions, diagnoses, and treatment plans
  • Foster trust and consult with patients, families, and caregivers during visits
  • Order diagnostic testing and prescribe medications, supplies, and interventions as medically necessary
Requirements
  • Board Certified, with a valid NP or PA certification in the state of New Jersey
  • Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose
  • Positive demeanor and strong people skills
  • Ability to confidently problem solve and operate independently
  • 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)
