POSITION SUMMARY: The Case Manager will be responsible for the initial assessment of incoming clients and their enrollment in the Wellness Early Intervention Program. This role involves delivering Case Management services in accordance with established protocols. The Case Manager will also facilitate Wellness Self-Management sessions and collaborate closely with the existing Care Coordinator and Health Navigator. Additionally, the Case Manager will provide essential connections to mental health and primary care services, initiate clients' Electronic Health Records (EHR), and conduct Rapid HIV testing as needed, referring positive results for confirmatory testing as appropriate.
KEY RESPONSIBILITIES:
- Manage client entry into the Wellness Early Intervention Program, ensuring compliance with service delivery standards.
- Initiate or update clients' EHR as necessary.
- Administer assessment tools during client intake and discharge.
- Conduct Wellness Self-Management and ARTAS sessions to support client success.
- Provide Rapid HIV testing services, ensuring adherence to all relevant regulations, including HIPAA.
- Identify potential issues and communicate them to the Program Director or the WEIP Nurse Practitioner/Registered Nurse.
WORK SCHEDULE: Monday to Friday, 8 AM to 4 PM
SALARY RANGE: $55,000 - $57,325 annually

QUALIFICATIONS:
- Bachelor's degree required; Certification in Case Management preferred.
- A minimum of two years' experience providing services to individuals with substance use disorders.
