We recognize the significance of fostering a comfortable and supportive environment, whether residents are with us for short-term rehabilitation or long-term care.
Key Responsibilities
- Conduct thorough admission interviews with residents and their families to compile a confidential psychosocial history and assessment, identify concerns, formulate treatment plans/goals, and finalize advance directives.
- Regularly update residents' charts and social services records every three months post-initial evaluation, or as social service needs arise. Reassess resident care plans at least quarterly.
- Maintain ongoing communication with families to keep them informed about residents' needs.
- Assist residents with personal needs such as clothing purchases, acquiring telecommunication services, and arranging transportation.
- Advocate for residents' rights in accordance with applicable regulations.
- Collaborate with relevant staff to address resident needs and concerns, providing necessary resources.
- Support discharge planning in partnership with the Supervisor and Director of Nursing.
- Participate in Resident Care Conferences, communicating resident and family concerns.
- Prepare confidential cases or issues for monthly meetings with the Social Work Consultant.
- Review Advance Directives and/or Preferred Intensity of Care with new residents and their families, ensuring follow-up on completion.
- Assist nursing staff with residents' dental, vision, hearing, and podiatry requirements.
