Crisis Residential Program services provide short-term intensive, round-the-clock, personalized support to help stabilize individuals experiencing a mental health crisis, preventing hospitalization. Our teams collaborate with existing treatment providers and connect clients to community services and resources offered by Sheppard Pratt or other community providers.
Key Responsibilities- Assess and monitor client needs, developing service plans and connecting clients to community services and resources.
- Coordinate client medical appointments, transportation, housing, money management, and medication management.
- Obtain and maintain entitlements for clients, ensuring access to necessary resources.
- Maintain accurate and complete documentation, including client records and transition plans.
- Develop and coordinate transitions plans and program discharge, ensuring a smooth transition for clients.
- A commitment to professional development, including a comprehensive tuition reimbursement program to support ongoing education and licensure.
