Our teams collaborate with existing treatment providers and connect clients to community services and resources offered by Sheppard Pratt or other community providers.

Key Responsibilities
  • Assess and monitor client needs, developing service plans and connecting clients to community services and resources.
  • Coordinate client medical appointments, transportation, housing, money management, and medication management.
  • Obtain and maintain entitlements for clients, ensuring access to necessary resources.
  • Maintain accurate and complete documentation, including client records and transition plans.
  • Develop and coordinate transitions plans and program discharge, ensuring a smooth transition for clients.

Requirements
  • A high school diploma or GED and 2 years of related experience, or a bachelor's degree and 1 year of related experience.

  • Availability to work days, nights, weekends, and holidays depending on program need.
