Job Description

Included Health is seeking a skilled Clinical Navigation, Nurse Practitioner/Physician Assistant to join our team. As a key member of our virtual care team, you will play a critical role in delivering high-quality, patient-centered care to our members across the country.

About the Role

This is a full-time position that requires 2,080 annual hours, with a schedule of 40 hours per week or 80 hours per pay period. The successful candidate will be required to work shifts from 8 AM - 5 PM or 9 AM - 6 PM Pacific Time, with minimal weekend work (4-6 times per year).

Key Responsibilities
  • Develop and implement personalized care plans to improve health outcomes for patients.
  • Provide clinical guidance and navigation to patients through various modalities, including phone, video, and secure messaging.
  • Collaborate with healthcare professionals and administrative staff to coordinate care and ensure service delivery aligns with overall organizational objectives.
  • Participate in continuous improvement initiatives aimed at optimizing efficiency and effectiveness of clinical services.
Requirements
  • Be a licensed NP/PA provider in good standing with your state board.
  • Maintain board certification.
  • At least 3 years of NP/PA experience in a clinical setting is required.
  • Willingness to obtain and maintain additional licenses as necessary.
  • Strong communication skills, with an emphasis on clear and detailed medical documentation.
  • High empathy and ability to reassure patients and families during challenging times.
  • Strong judgment and efficiency in balancing priorities.
  • Flexibility to work cross-functionally in a fast-paced environment.
Compensation and Benefits

The United States base salary target range for this full-time position is $136,113 - $176,950, depending on location and experience. This includes equity and benefits. We also offer a comprehensive compensation package, including remote-first culture, 401(k) savings plan, comprehensive medical, vision, and dental coverage, and generous paid time off.
