Trustbridge is seeking a compassionate and skilled LPN to join our team as a Hospice Care Partner. As a key member of our interdisciplinary team, you will provide end-of-life care to patients and their families, ensuring their comfort and quality of life.
Responsibilities- Coordinate patient care under the direction of the primary physician and registered nurse.
- Identify patient and family needs, and coordinate care accordingly.
- Contribute to the development and implementation of individualized plans of care.
- Utilize monitoring, intervention, and teaching skills to maximize patient comfort and enhance quality of life.
- Pledge to prioritize patient comfort and care above all else.
- Current license to practice as a Licensed Practical Nurse in the State of Florida.
- Current Basic Life Support Certification.
- 30 hour IV Certification preferred.
- Hospice Certification preferred.
- Ability to communicate effectively with patients, families, and healthcare professionals.
- Bilingual (English/Spanish) preferred.
- Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
We offer a competitive pay range of $20/hr. - $27/hr. based on experience. If you are a caring and detail-oriented individual who is passionate about providing exceptional patient care, we encourage you to apply.
