Job Summary

We are seeking a highly skilled Early Intervention Professional to join our Part C- Early Intervention Team under our Developmental Services Division. The successful candidate will provide service coordination and direct clinical service to clients, including determining eligibility, assessing client needs, developing treatment plans, and providing prevention services.

Key Responsibilities
  • Assist with screening for eligibility of children referred to Part C for services and make recommendations for services based on clinical impression.
  • Conduct comprehensive developmental assessments and diagnostic impression upon admission for services within a multidisciplinary team.
  • Monitor and manage an assigned caseload.
  • Assist families in identifying goals and jointly prepare individualized family service plan (IFSP).
  • Assess and monitor consumer needs and adjust service plan accordingly.
  • Interface with other service providers and arrange for additional/alternative services if appropriate.
  • Provide individual or group therapy; implement IFSP; coordinate daily treatment schedule.
  • Provide crisis intervention and prevention services to individuals and families as needed.
  • Provide consultation, education and prevention services.
  • Conduct Service Coordination services as needed; coordinate and link client to community resources and medical services; interface with other service providers and client support system.
  • Document and chart information about client assessment, progress notes, and Initial, 6-month, requested and annual IFSPs, discharge summaries and other client related data, correspondence, medical records, etc in accordance with regulatory compliance.
  • Maximize reimbursement.
  • Attend professional conferences and trainings as required.
  • Supervise student interns/ externs as needed.
Requirements
  • Master's degree in a discipline meeting qualifications as an Early Intervention Professional outlined in the Infant and Toddler Connection of Virginia's Practice Manual.
  • At least one year of experience working with young children with developmental delays.
  • Discipline specific licensure as outlined in Infant and Toddler Connection of Virginia's Practice Manual.
  • Valid driver's license in the Commonwealth of Virginia.
