Job Title: Home Health LPN

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in providing high-quality patient care in the comfort of their own homes.

Job Summary:

The LPN will be responsible for delivering personalized care to patients with diverse needs, working closely with our interdisciplinary team to develop and implement effective care plans.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions.
  • Collaborate with patients' families and caregivers to educate them on care techniques and promote a safe and healthy environment.
  • Document patient progress and communicate effectively with the healthcare team to ensure seamless care coordination.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current LPN licensure in the state of practice.
  • Valid driver's license, vehicle insurance, and access to a reliable vehicle or public transportation.
  • CPR certification.
