Job Summary

We are seeking a skilled Home Health Licensed Practical Nurse (LPN) to provide quality patient care in the patient's home. The ideal candidate will have a strong background in nursing and excellent communication skills.

Responsibilities
  • Provide technical skilled care in the patient's home, in accordance with the plan of care and in coordination with other members of the healthcare team.
  • Continually observe and report the patient's nursing needs, instructing the patient's family in the care of the patient and maintenance of a healthy environment.
  • Complete progress notes for each visit and submit them to the agency daily.
  • Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition.

Requirements
  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
