Job Title: Clinical Pharmacist

At DHR Health, we are seeking a highly skilled and dedicated Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will play a vital role in ensuring the safe and effective use of medications in our healthcare setting.

Responsibilities:
  • Review and interpret medication orders to ensure accuracy and appropriateness.
  • Provide clinical consultation and guidance to healthcare professionals on medication therapy and management.
  • Monitor and evaluate patient medication regimens for potential interactions, allergies, and adverse effects.
  • Develop and implement medication use policies and procedures to promote safe and effective medication use.
  • Collaborate with healthcare teams to develop and implement patient care plans that incorporate medication therapy.
  • Provide education and training to healthcare professionals on medication-related topics.
  • Participate in quality improvement initiatives to enhance medication use and patient outcomes.
Requirements:
  • PharmD or BS Pharmacy degree from an accredited institution.
  • Current licensure to practice pharmacy in the state of Texas.
  • Minimum of 2 years of experience in a hospital or healthcare setting.
  • Strong clinical and communication skills.
  • Ability to work effectively in a fast-paced environment.
