Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at DHR Health. As a Clinical Pharmacist, you will play a critical role in ensuring the safe and effective use of medications in our healthcare setting.
Responsibilities:- Review and interpret medication orders to ensure accuracy and appropriateness.
- Provide medication information and advice to healthcare professionals and patients.
- Assess and monitor patient medication therapy to prevent adverse reactions and optimize treatment outcomes.
- Supervise and direct pharmacy support personnel to ensure efficient and effective medication management.
- Collaborate with healthcare teams to develop and implement medication use policies and procedures.
- Participate in medication use review activities to identify opportunities for improvement and optimize medication use.
- BS Pharmacy degree or Pharm.D. or equivalent required.
- Licensed to practice pharmacy in the state of Texas required.
- Completed all continuing education requirements.
- Two (2) years of hospital experience preferred, but not required.
- Twenty hours of IV admixture preparation training.
The individual spends over 95% of their time in an air-conditioned environment with varying exposures to noise. There is protection from weather conditions but not necessarily from temperature changes.
The position does have low exposure to malodourous, infectious body fluids from patients and some minimal exposure to noxious smells from cleansing agents.
This position has minimal to no exposure to blood, body fluids, or tissues and is an OSHA Category III.
