About the Role

The Behavioral Health Social Worker is an integral member of our collaborative care team at Galileo. Reporting to Behavioral Health Leadership, you will work directly with patients and side by side with primary care providers and health advocates to craft, facilitate, and maintain care plans tailored to patients' needs and goals.

As a subject matter expert within the field of behavioral health, you will be the primary liaison with external organizations in the community to facilitate adherence to the care plan and educate patients/family caregivers on how to navigate the healthcare system.

This role requires direct patient consultation, understanding who they are as people, identifying their social needs, and matching them to relevant resources. You will also collaborate closely with the team to craft personalized care plans, provide support and guidance to an interdisciplinary care team, and conduct psychosocial evaluations.

Key Responsibilities:

  • Consult directly with patients to identify social needs and match them to relevant resources.

Travel Requirements: 60% of the time spent in the field seeing patients in Syracuse, NY and surrounding counties. The rest of the time will be remote work.

Compensation: $72,000 per year (estimated), with benefits including medical, dental, and vision insurance, flexible spending account, health savings account with match, company-paid STD/LTD, AD&D, and life insurance, paid family leave, paid time off, and 401(k) with match.
