Job Description for Supportive Care Associate
Essential Qualifications:

Must possess a High School Diploma or Equivalent.
Criminal Background Check or proof of rehabilitation is necessary.
Availability for 8-hour shifts every other weekend is required.
Must be free from communicable diseases.
Position Purpose: To deliver a secure, high-quality, professional, and compassionate service. You will be tasked with fostering the highest level of independence and improving the quality of life for all individuals participating in Abilities, Inc. programs. This role is designed to ensure the utmost welfare, safety, and security for you, the community, and the clients we serve.
Job Responsibilities:
- As a member of Abilities, Inc., you may be required to rotate among various facilities and program supervisors. Flexibility with these changes is expected as you are an employee of Abilities, Inc. and not assigned to any specific location.
- Oversee residents in accordance with their Individual Service Plans (ISP), Behavioral Support Plans (BSP), and Behavioral Intervention Plans (BIP). Employees must also be knowledgeable about and review Elopement Plans/Protocols, Seizure Protocols, Diabetic Protocols, and any other specific requirements pertaining to the residents under their supervision.
- Serve as a positive role model. Utilize positive reinforcement, avoid demeaning language, and never engage in actions that contradict the best interests of the residents of Abilities, Inc.
- Maintain a clean and welcoming facility. Ensure that the environment is homelike and encourage residents to develop the skills necessary to maintain cleanliness in their living spaces. It is your responsibility to guarantee that residents have a tidy and hazard-free environment, while also being mindful of fire hazards in their rooms.
- Exhibit a professional demeanor at all times towards residents, their guests/family members, guardians, care managers, supervisors, and fellow employees of Abilities, Inc. Demonstrate respect and consistency with program structures.
- Complete all tasks assigned by supervisors.
- Prepare meals according to the posted menu and adhere to sanitary practices. Prepare special diets as directed by the resident's physician. Grocery shopping and menu planning may be part of your duties. Encourage resident involvement in all aspects of meal planning and preparation.
- Deliver Personal Care Services as specified in the residents' Individual Service Plans (ISP). This may involve lifting and transferring residents, requiring the use of gait belts, adaptive equipment, and the ability to lift up to 100 pounds. Please familiarize yourself with Abilities, Inc. Limited Lift Policy.
- Plan and engage in community and in-facility activities. An activity calendar will be consistently available in the facility.
- Become acquainted with Evacuation Procedures, Grievance Procedures, and Program Statements, which are displayed in the facility. Review all Policies and Procedures outlined in the Employee Handbook.
- Attend all scheduled shifts and staff meetings. Adhere to Abilities, Inc. Attendance policy and take responsibility for your shifts.
- Communicate comprehensive and accurate reports through both verbal and written skills. Proficiency in electronic charting and basic computer knowledge is required.
- Provide safe transportation for residents as directed by your supervisor for work, community activities, personal care shopping, Adult Day Care services, and medical appointments.
- Administer and monitor medications as prescribed by the resident's physician. Gain knowledge of potential side effects and interactions of the medications. This can be assessed through Extended Care Professional (ECP) and Abilities, Inc. electronic charting for all medication administration and resident documentation. You will also be responsible for shift-to-shift Schedule 2 medication counts as an approved medication passer/monitor.
- Familiarize yourself with the State Requirements outlined in DHS 88 and DHS 83. Copies will be provided at the facility for your review.

This includes your required 15 hours of Continuing Education (CEU Training) after your annual hire date. Failure to complete any training within the required timeline may result in suspension and/or termination of employment.
- Report any maintenance issues to your program supervisor immediately; you will need to complete a maintenance request form.
- Report any changes in condition to your program supervisor without delay and complete the necessary documentation.
- Log in and out when reporting to and leaving work. You will be accountable for reviewing your hours and reporting any discrepancies to your direct supervisor as soon as you notice an error.
- Report any workplace injuries immediately to your direct supervisor and complete the required paperwork.
- Undergo an initial TB (Tuberculosis) skin test, which must be placed and read after 72 hours, prior to starting work. A second TB skin test will be conducted within two weeks of the initial test.
