At Elara Caring, we are dedicated to providing exceptional home care services to our patients. As a Private Duty Pediatric LPN, you will play a vital role in delivering quality care to our clients in the comfort of their own homes.

Key Responsibilities:

  • Provide comprehensive nursing care to clients in their homes under the direction of a registered nurse.
  • Participate in the development of the plan of care along with other Interdisciplinary team members and report to the Field Nursing Supervisors.
  • Provide health education to patients and their caregivers.
  • Responsible for accepting and entering verbal and written orders from physicians as required; obtain co-signature of RN Case Manager on all orders.
  • Follow up with physicians, clients, family members, staff, and RN on client problems and concerns. Maintain communication with physicians including report of client condition change.
  • Complete accurate and timely documentation of care provided.

Requirements:

  • Must have an active nurse license in the state of Connecticut or compact license.
  • Minimum 6 months LPN Pediatric experience; LPN Home Health experience preferred.
  • Reliable transportation to perform job duties.
  • Dedication to quality patient care.

Reporting Structure:

You will report to the Clinical Team Manager.
