Pediatric Home Health Nurse

Delivering Quality Care in the Comfort of Home
At Elara Caring, we are dedicated to providing exceptional home care services to our patients. As a Private Duty Pediatric LPN, you will play a vital role in delivering quality care to our clients in the comfort of their own homes.
Key Responsibilities:
- Provide comprehensive nursing care to clients in their homes under the direction of a registered nurse.
- Participate in the development of the plan of care along with other Interdisciplinary team members and report to the Field Nursing Supervisors.
- Provide health education to patients and their caregivers.
- Responsible for accepting and entering verbal and written orders from physicians as required; obtain co-signature of RN Case Manager on all orders.
- Follow up with physicians, clients, family members, staff, and RN on client problems and concerns. Maintain communication with physicians including report of client condition change.
- Complete accurate and timely documentation of care provided.
Requirements:
- Must have an active nurse license in the state of Connecticut or compact license.
- Minimum 6 months LPN Pediatric experience; LPN Home Health experience preferred.
- Reliable transportation to perform job duties.
- Dedication to quality patient care.
Reporting Structure:
You will report to the Clinical Team Manager.
