About the Role

Solvista Health is seeking a skilled Care Navigator to join our Crisis Response Team. As a Care Navigator, you will play a vital role in supporting our team by providing a partnership approach to healthcare and community-based services.

Key Responsibilities
  • Coordinate and monitor provider services and care for a specified client population, including home and/or school visits, physician/hospital visits, and therapy visits.
  • Assess client needs through thorough interviews and document all client encounters and contracts made on behalf of clients/families.
  • Evaluate and document the progress of individual therapeutic programs and make modifications as required.
  • Facilitate client/family access to healthcare, school, and community resources, and assist in developing natural resources and connecting them with support networks.
  • Research and understand community resources, and travel throughout our service area to meet with clients/families, service providers, and community partners.
  • Work with clients and/or families to assess, develop, implement, monitor, and modify plans, and perform other duties as assigned.
Requirements
  • Bachelor's degree in social work, counseling, psychology, or a related healthcare field from an accredited institution.
  • Ability to develop, implement, and modify multidisciplinary healthcare plans, and knowledge of billing procedures for clinical and social services.
  • Strong interpersonal and communication skills, and ability to work effectively in diverse populations.
Working Conditions

This role operates in a fast-paced, professional office environment and requires the use of standard office equipment. The position may require travel to conferences, meetings, and branch locations on a regular or intermittent basis. Work may involve moderate exposure to unusual elements, such as extreme temperatures, dirt, dust, fumes, smoke, unpleasant odors, and/or loud noises.

This is a largely sedentary role but can involve standing or sitting for extended periods of time, bending at the waist, and using hands and fingers to handle and file papers or operate assigned equipment. While performing the duties of this job, the employee is regularly required to talk or hear. Specific vision abilities required by this job include close vision, distance vision, color vision, and ability to adjust focus. Employee may also have to lift 10 to 25 lbs. unassisted.

Conditions of Employment
  • Annual TB, federally required drug screening, and Influenza vaccination or compliance with policy and procedure.
  • Required possession of a valid state driver's license.
  • Successful candidate must submit to and pass post-offer drug screen.
