Responsibilities
  1. Perform comprehensive patient evaluations, including history and physical assessments
  2. Develop and implement individualized treatment plans, taking into account patient needs and goals
  3. Provide education and support to patients and families on health promotion and disease management
  4. Collaborate with healthcare professionals to ensure coordinated care
  5. Maintain accurate and timely documentation in the electronic medical record
Requirements
  • Master's degree from an accredited Nurse Practitioner program
  • 1-2 years experience as a Nurse Practitioner in an outpatient ambulatory care setting
  • Current license to practice as a Nurse Practitioner in New York State
