Key Responsibilities:
As a Hospice RN Case Manager, you will:
1. Develop and implement personalized care plans for patients, ensuring all necessary services are identified and delivered.
2. Collaborate closely with the Interdisciplinary Group, patients, families, and attending physicians to coordinate care.
3. Conduct thorough nursing evaluations and assessments to monitor patient progress.
4. Provide guidance and supervision to LVNs and Home Health Aides involved in patient care.
5. Manage cases effectively and perform additional duties as required.

Qualifications:
To be successful in this role, you should possess:
- A valid Registered Nurse license.
- At least one year of nursing experience.
- Preferred experience in hospice care, though not mandatory.
- A valid driver's license and reliable transportation.
- The ability to lift up to 50 pounds.
- A clear background check and completion of required health screenings.
