Compensation: Competitive hourly wage based on experience.
Sign-on Bonus: A monetary incentive available after a specified period of employment.
Work Schedule: Day shifts, three 12-hour shifts per week.

Key Responsibilities:
  • Deliver personal care in accordance with the established care plan, ensuring patient privacy and comfort.
  • Evaluate the patient's capacity to undergo treatment prior to care initiation and monitor their condition post-care.
  • Collaborate with care managers or registered nurses to maintain and update care plans, documenting any changes in patient status.
  • Assist patients with personal care, mobility, and positioning, while guiding family members or caregivers in providing care.
  • Prepare and serve meals in accordance with established guidelines, assisting with feeding as necessary, and perform essential housekeeping tasks to ensure a safe environment for patients and families.
Qualifications:
  • Current CNA certification.
  • Ability to successfully complete background checks, drug screenings, and health assessments.
  • Graduation from an accredited nursing program.
  • Preferred experience of 1-2 years in a medical care setting, particularly in home health or hospice.
  • Valid automobile insurance, Oregon driver's license, and reliable transportation.
  • Current CPR certification.
  • Excellent communication and interpersonal skills.
  • Ability to work both independently and as part of a collaborative team.
  • Demonstrated compassion, empathy, and commitment to patient care.
