Terraces of Boise, part of the esteemed HumanGood network recognized by U.S. News & World Report for excellence in skilled nursing and continuing care retirement communities, is seeking an Admissions / Unit Charge RN.
** Competitive pay range: $41 - $46/hr. (based on experience) ** Work schedule: Primarily daytime hours with some flexibility based on admissions needs.
Key responsibilities may include:
- Collaborating with hospital case managers, patients, and their families to facilitate placement for skilled nursing services or rehabilitation therapy.
- Engaging with the interdisciplinary team (IDT) to ensure a seamless transition from various healthcare settings to our Skilled Nursing Health Center.
- Overseeing the inquiry process and managing reviews through the Referral Manager System to support the admissions team.
- Assisting the admissions counselor with inquiries from families, hospitals, assisted living facilities, and other referral sources.
- Maintaining strong communication and relationships with hospital accounts, accountable care organizations (ACOs), and other referral partners.
- Ensuring adherence to the standard admission process via the electronic health record (EHR) system; managing incoming continuity of care documents (CCDs) through EHR integration.
- Communicating specific needs of new admissions to the IDT, primary care physicians, or other providers to ensure quality, person-centered care and support discharge objectives.
- Participating in care conferences, assisting with appointments, and fulfilling other related nursing duties as required.
To excel in this position, you should possess:
- A valid RN license in the state of application
- Current CPR certification
- Previous nursing experience in senior care, skilled nursing, admissions, or post-acute care is preferred
