Matrix Providers is seeking a dedicated Pharmaceutical Care Specialist to become part of our skilled team that delivers healthcare services to Military Service Members and their families.

This position offers hourly pay, disbursed bi-weekly.

Schedule:
Monday - Friday

Qualifications:
The Pharmaceutical Care Specialist must fulfill the following criteria:

Degree:
Doctor of Pharmacy (Pharm.
D.)

Education:
Graduate from an institution accredited by the Accreditation Council on Pharmaceutical Education (ACPE).

Internship/Residency:

Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health-System Pharmacists or American College of Clinical Pharmacy, or documentation of relevant education, training, and continuing medical education in clinical pharmacy practice.


Experience:
Must meet clinical competency requirements as specified in the Service-specific credentialing instructions.

Possess one (1) year of full-time experience within the last three (3) years as a clinical pharmacist in an ambulatory care environment OR have completed an ASHP accredited Ambulatory Care Residency OR hold Board Certification as a Pharmacotherapy Specialist.


Licensure:
Must hold a current, full, active, and unrestricted license to practice as a pharmacist.

Job Overview:
Evaluate patients' responses to medication therapy and develop treatment plans based on physician-established diagnoses.
Order and assess laboratory tests necessary to monitor the effects and outcomes of drug therapy.

Initiate, adjust, or discontinue medications for the ongoing management of chronic conditions (e.g., hypertension, hyperlipidemia, anticoagulation, diabetes, asthma, refill clinics, etc.) in collaboration with medical staff.

Oversee and manage pharmacotherapy that may require periodic adjustments due to specific or changing pharmacokinetic characteristics (e.g., aminoglycosides, phenytoin, antithrombotic).
Administer prescription or non-prescription medications in accordance with established protocols.
Assess metabolic requirements and order therapeutic nutrition products in inpatient settings.
Review medical and medication histories for drug-related issues and modify drug therapy as needed.
Collaborate with other healthcare professionals (e.g., physicians, dietitians, nurses, physical therapists, etc.) regarding patient treatment needs or options.
Conduct a comprehensive range of pharmacist procedures.
Review patient profiles during dispensing to closely monitor medication therapy.

Ensure prescription instructions, dosage, medication interactions, therapeutic duplications, allergy information, age-specific dosages, and other relevant data are appropriate for each patient.

Guarantee the accuracy of all pharmaceutical products prepared or processed by technical staff.

Provide training to clinical staff on new medications, appropriate medication use, and comparisons of current therapies, as deemed necessary by the department head.

Consult with specialty practitioners to ensure optimal pharmaceutical care for patients.
Document significant medication interactions and pharmacy interactions with prescribers.

Conduct Adverse Drug Reaction (ADR) reporting in accordance with The Joint Commission (TJC) or the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC) standards as appropriate.
