About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team in Humboldt County. As a LPN, you will work alongside an interdisciplinary team to provide high-quality care to patients in their homes.

Key Responsibilities

Requirements
  • Current LPN licensure in the state of California.
  • 1 year of experience as a Licensed Practical Nurse, preferably in home health or a related setting.
  • National Provider BLS certification.
  • Ability to drive and provide transportation for patients, as needed.
