Job Summary

We are seeking a highly skilled and compassionate Social Services Coordinator to join our team. As a key member of our staff, you will play a vital role in linking our residents to the supportive services they need to maintain their independence and quality of life.

Key Responsibilities
  • Provide comprehensive case management services to residents, including intake, assessment, and referral to community resources.
  • Establish and maintain relationships with local service providers to ensure seamless access to necessary services.
  • Develop and implement individualized care plans that meet the unique needs of each resident.
  • Monitor and evaluate the effectiveness of services provided, making adjustments as needed to ensure optimal outcomes.
  • Collaborate with other staff members to ensure a comprehensive approach to resident care.
Requirements
  • Bachelor's degree in a related field, such as health services or social work.
  • Minimum of three years of experience in a related field, preferably in a nursing home or healthcare setting.
  • Strong knowledge of medical terminology, physical conditions, and healthcare delivery systems.
  • Ability to assess and meet resident needs, with a focus on aging and geriatrics.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, staff, and community partners.
