About the Opportunity

Optum Home & Community Care is seeking a skilled Advanced Practice Clinician or Physician to join its team. As a member of our HouseCalls product, you will provide annual clinical assessments to patients in their homes, helping to identify and reduce health risks, coordinate follow-up care, and improve overall well-being.

This role offers a dynamic work environment that values work-life balance, with flexible scheduling options available between 20-40 hours per week. You will have the ability to achieve a better balance between your professional and personal life without the burden of on-call shifts, weekends, or holidays.

The role requires travel across New Mexico, approximately 85% of the time. All travel expenses are covered, and you will receive a competitive salary plus multiple incentives.

Your Key Responsibilities:

  • Conduct in-home assessments, including evaluations of past medical history, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate.
  • Identify diagnoses for care management and active medical management in furtherance of treatment.
  • Formulate lists of current and past medical conditions using clinical knowledge and judgment, as well as assessment findings.
  • Communicate assessment findings to inform members' Primary Care Physicians (PCPs) of potential gaps in care.
  • Recognize urgent and emergent situations and intervene appropriately.
  • Educate members on disease processes, medications, and compliance.
  • Travel up to 85%.

Requirements:

  • An active NP or PA license or the ability to obtain by start date, with licensure unencumbered and free of any open/unresolved disciplinary actions.
  • Active national NP or PA certification or the ability to obtain national certification in the state of assignment by start date.
  • The ability to complete the physical requirements of the job, including lifting a 30-pound bag, navigating stairs and various dwelling conditions, and sitting, standing, and kneeling for physical assessments.
  • Access to reliable transportation for completing home visit assessments.
