About Our Role

We are seeking an experienced Advanced Practice Clinician or Physician to join our HouseCalls team at Optum Home & Community Care. In this role, you will conduct in-home assessments, providing critical healthcare services to patients in need.

This opportunity offers a unique blend of work-life balance and flexibility, allowing you to choose from a range of schedules between 20-40 hours per week. You will have the autonomy to prioritize your workload and manage your time effectively, ensuring a healthier balance between your professional and personal life.

As a member of our team, you can expect to spend approximately 85% of your time traveling across New Mexico, conducting home visits. Don't worry about the commute – all travel expenses are covered, and you'll enjoy a competitive salary plus additional incentives.

Key Responsibilities:

  • Conduct comprehensive in-home assessments, including thorough evaluations of patients' medical histories, medication regimens, and vital signs.
  • Develop and implement personalized care plans, tailored to each patient's unique needs and circumstances.
  • Collaborate with Primary Care Physicians (PCPs) to ensure seamless coordination of care and optimal patient outcomes.
  • Utilize evidence-based practices to identify and address potential health risks, promoting proactive healthcare and prevention.
  • Provide education and support to patients, empowering them to take control of their health and well-being.
  • Travel up to 85% of the time, enjoying a competitive salary and generous benefits package.

Benefits Package:

  • A competitive base salary of $125,000-$145,000 per year, based on experience.

Requirements:

  • An active NP or PA license or the ability to obtain by start date, with a clean record and no outstanding disciplinary actions.
  • Current national NP or PA certification or the ability to obtain certification in the state of assignment by start date.
  • The ability to meet the physical demands of the role, including lifting heavy equipment and navigating varying environmental conditions.
  • Reliable transportation for conducting home visits and accessing remote areas.
