About the Role

Pacific Clinics is seeking a highly skilled Family Partner II to join our team. As a Family Partner II, you will work closely with families and caregivers to provide culturally responsive support and guidance. Your expertise will help families navigate complex systems and access necessary resources.

Key Responsibilities
  • Provide peer support to parents and caregivers, utilizing your personal and professional experience to establish credibility and trust.
  • Model non-judgmental, culturally relevant support to parents and caregivers, and provide training to staff on best practices.
  • Collaborate with internal teams to identify community connections and resources for families.
  • Coach staff on how to explore, identify, and re-establish natural support systems for families.
  • Participate in Child and Family team (CFT) meetings to ensure access, voice, and choice within the process.
Requirements
  • High School diploma or GED required.
  • Up to three (3) months of experience as a primary caregiver of a child with complex needs.
  • Ability to articulate the experience and perspective of a parent/caregiver or relative of a child with complex needs.
Preferred Qualifications
  • Some college education is preferred.
