The Support Services Specialist is an integral member of a multidisciplinary team dedicated to delivering exceptional care and facilitating engaging activities in a secure and nurturing environment, both at home and within the community.
KEY RESPONSIBILITIES:
- Assist clients with essential daily living skills and personal tasks, including but not limited to hygiene, dressing, household chores, grocery shopping, meal preparation, and financial management.
- Provide transportation for clients to various activities and appointments, necessitating a valid driver's license.
- Ensure effective communication regarding client needs and progress through appropriate channels, such as facility logs and staff meetings, by the end of each shift.
- Complete all required documentation accurately, including Behavioral Reports, Incident Reports, and Progress Reports, in compliance with organizational standards.
- Maintain meticulous records for transportation vehicles and adhere to Medicaid guidelines for billing and mileage.
- Report any incidents or emergencies to the supervisor or designated authority immediately, as this role is safety-sensitive.
- Contribute to the development, management, and revision of clients' Care Plans to prioritize their health, safety, and well-being.
- Follow medication management protocols and assist in monitoring and documenting these processes.
- Organize and facilitate recreational activities for clients.
- Exemplify positive role modeling for clients through professional appearance and behavior, adhering to the organization's dress code.
- Treat clients and their families with respect and dignity, reflecting the mission of Friendship Community Care in all interactions.
- Assist in maintaining the cleanliness and functionality of organizational facilities and vehicles.
- Participate in required training and continuing education opportunities.
- Provide proof of liability insurance and keep it current.
- May be required to undergo periodic driving assessments and demonstrate proficiency in operating various vehicles.
- Maintain confidentiality regarding all information related to Friendship Community Care and its clients, in accordance with HIPAA policies.

QUALIFICATIONS:

EDUCATION AND EXPERIENCE:
A high school diploma or GED is required; experience working with individuals with developmental disabilities is preferred. A valid driver's license is also necessary.

Ability to read and comprehend simple instructions and correspondence. Proficient in writing basic correspondence and effectively presenting information in one-on-one and small group settings.
MATHEMATICAL SKILLS:
Ability to perform basic arithmetic operations and interpret data presented in various formats.
REASONING ABILITY:
Ability to apply common sense to follow instructions and interpret various forms of information.
PHYSICAL DEMANDS:
Employees must be able to perform essential functions, which may include lifting up to 100 pounds and engaging in various physical activities.
WORK ENVIRONMENT:
The work environment is typically moderate in noise level. This role is safety-sensitive and requires passing background checks and maintaining a clean driving record.
