Outpatient Primary Care Physician

The Brockton Neighborhood Health Center is seeking a dedicated medical professional to deliver comprehensive care to our patients in alignment with established protocols and standards. KEY RESPONSIBILITIES:
- Clinical Duties: Provide direct patient care services, including assessment, planning, documentation, and evaluation of patient needs and treatments. Ensure appropriate follow-up care.
- Cultural Competence: Maintain a practice that is sensitive to diverse cultural and linguistic backgrounds, utilizing interpreters and other resources as necessary.
- Care Planning: Develop individualized care plans based on thorough assessments of physical, emotional, and social needs, collaborating with patients and their families when appropriate.
- Referrals: Make necessary referrals to specialists and other healthcare providers as needed.
- Chart Review: Regularly review patient records to ensure adherence to treatment plans.
- Collaboration: Work alongside other healthcare professionals to promote patient education and rehabilitation, fostering both mental and physical health.
- Teamwork: Maintain a collaborative relationship with fellow physicians, administrative staff, and clinical personnel, providing guidance and support as needed.
- On-Call Duties: Participate in on-call rotations and provide coverage for urgent care services as required.
- Quality Improvement: Engage with the Patient Care Committee to enhance the quality of care delivered to patients.
- Patient Volume: Full-time physicians are expected to manage a minimum of 4200 patient visits annually, subject to adjustments based on compensation plans.
COMMUNICATION SKILLS:
- Exhibit sensitivity and respond effectively to both verbal and non-verbal cues from patients, families, and colleagues.
- Uphold patient confidentiality and dignity in all communications regarding care.
- Problem-Solving: Demonstrate sound judgment in resolving clinical and non-clinical issues.
- Possess the ability to work independently and make informed decisions.
- Anticipate potential challenges and contribute to problem-solving efforts.
INTERPERSONAL SKILLS:
- Display tact, patience, and discretion in interactions with others.
- Maintain composure under the pressures of a demanding work environment.
PROFESSIONAL DEVELOPMENT:
- Actively participate in scheduled staff meetings and professional development opportunities.
- Engage in both formal and informal teaching of staff members.
- Commit to ongoing medical education and training to ensure clinical competence.
- Maintain admitting privileges at affiliated medical facilities.
MINIMUM QUALIFICATIONS:
- Possess a valid Massachusetts medical license and relevant certifications.
- Board certified or eligible in family practice or internal medicine, with a commitment to pursuing board certification if not yet achieved.
- Ability to collaborate effectively with a multidisciplinary team, including mid-level providers, social workers, and case managers.
- Fluency in Spanish, Portuguese, French, or Haitian Creole is highly desirable.
- Prior experience in ambulatory care or public health is preferred.
WORK ENVIRONMENT:
- Long hours may be required, including participation in rotating on-call duties.
- Potential exposure to patient blood and bodily fluids.
Physical requirements include the ability to lift equipment and patients weighing up to 50 pounds, frequent standing and walking, and sufficient visual and auditory acuity for effective communication. Brockton Neighborhood Health Center is committed to ensuring a safe and healthy environment for both patients and staff.
