Job Description

**Home Health Nurse - Travel Position**

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Nurse in a travel position. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities:

  • Conduct thorough assessments of patients' physical, emotional, and social needs
  • Develop and implement individualized care plans to meet patients' unique needs
  • Provide skilled nursing care, including wound care, medication management, and education
  • Collaborate with other healthcare professionals to ensure seamless care coordination
  • Document patient progress and outcomes in a timely and accurate manner

Requirements:

  • Current RN license in the state of Pennsylvania
  • Minimum of 1 year of recent home health nursing experience
  • OASIS/Medicare Certified Visits experience
  • Electronic Charting experience
  • American Heart Association BLS certification
  • Valid U.S. Driver's License
